EMS User Account Request

In order for your request to be processed, please fill out all sections of the form below and press the submit button at the bottom of this page.  Completion of this form will not guarantee acceptance of your event by Conference Services at San Francisco State University. 

All pending user account requests must be approved by Conference Services Events Office.  Request are reviewed daily, if your account is not approved within 24 hours please call the Events Office at (415) 405-4427 or email

Student organizations must be verified and approved by LEAD (Leadership Engagement Action & Development). 

A confirmation email will be sent to you after your request has been reviewed.  If there are blank areas on this form your request will be denied.

Once you have received an email confirming your user account, you may log in and start making reservations.

Please note your email address will be your User Id when signing into your account!
